HOW TO ORDER ID CARDS ONLINE
Please NOTE: We do NOT keep a database of or store any information parents provide. Enter as much or as little information as you'd like. Keep in mind that the more information you provide, the better in the event of an emergency.
Each 911 Kids Club ID card contains vital information kids or parents can hand to First Responders if the child is critically injured or gets lost. The ID card:

Provides critical medical and lifesaving info on existing medical conditions and allergies to pass to First Responders in case of injury to the child

Features 17 fields of information in total, including parent contact information, doctor’s name, and physical characteristics of the child

Encourages children to be more responsible for their safety, helping them to make better decisions
If you're unable to attend a local event to pick up an ID card for your child, order one online!
Only REQUIREMENT: your child is newborn to 16 years of age.
We process and mail all ID card requests weekly.
Provide the information that will be appear on each ID card. Get started below.
Upload a recent photo of your child. We accept JPG files. The photo should show the child's head and shoulders. Each photos should be approximately 1 5/8" H x 1" W (292 pixels W x 355 pixels H).
Make a small donation to cover shipping/handling (we recommend $2 for 1 set of child/parent cards). We also welcome larger donations to help us provide this valuable service to even more kids.
